Vinyl Siding Poorly Installed.

Question: We built a house about 5 years ago, and have Heartland siding. I am starting to suspect we had a poor installation done. We have had it blow off one side of the house a few times now in heavy winds. Each time we are the only house that loses any siding. Now the builder says that they can't replace it for us because the installer is out of business. My question is how many nails should be put in per piece. On that side of our house there is a foam inulation board on the whole wall. The siding is attached with staples across the small piece of vinyl on the top of the slots. But there is only a few staples in each piece because of the foam board they are only stapled where there is a two by four under the foam board. The siding that comes off is ripped right where the staples are. It seems like there should have been "furring strips" or something for the dsiding to be attached more securely. Am I right?
